The Claim Process

Anthem Claim Management will confirm receipt of a claim via email or USPS (if no email was provided) within 30 days of receipt.  If additional information is needed to process a claim, a request will be sent via email or USPS (if no email was provided).

Claims are processed in the order received. Federal law requires an interstate moving claim be processed within 120 days of receipt. Most claims are processed by Anthem Claim Management within 45-90 days of receipt, depending on seasonal volume.

Once a claim is reviewed by an adjuster, a summary of the determination and a settlement offer will be sent by USPS to the address on file.

What You Need to File

Please have the following information available before entering the the Claims portal.

Name of your Moving Company (Required to File a Claim)
The name of your moving company can be found on the top of your Bill of Lading and other documents received during the move.  You may have booked your move through a moving broker. If so, please remember that we need the legal name of your moving company (The company who executed your move) not the moving broker.

The Job or Reference Number for Your Move
This can be found on the upper right or left corner of your Bill of Lading and other documents received during the move.

A Copy of the Bill of Lading (contract)
If available, please supply a copy of Bill of Lading you received at pickup and a copy of the Bill of Lading signed at the time of delivery. Click for more information.

A Copy of your Household Goods Descriptive Inventory
If you moved locally, an inventory may not have been taken and is not need to file a claim. If available, please supply a copy of inventory you received at pickup and a copy of the inventory you received at the time of delivery. Click for more information.

Pictures of the Damage
For each item claimed, provide pictures of the damage, including at least one picture showing the entire item with the damage visible. Images should be renamed to identify the item depicted. Pictures are accepted in JPG, JPEG and PNG formats and should clearly show the damage claimed.

Manufacturer Model Number for Large Appliances, TVs and Vehicles 

Additional Documentation You Wish to Provide in Support of Your Claim

Note for Poor Quality or Missing Documents: Submit copies of the bill of lading and/or inventory even if they are hard to read. If you do not have a copy of the bill of lading and/or inventory you will have an opportunity to let us know.
